Power of attorney for licence application
Lets our maritime agent handle the issuing of your licence.

I appoint Tsutomu Kurita, maritime procedure agent, as my representative, and entrust to him all matters concerning applications under the Ship Officers Act and related laws, and the receipt of my small craft operator licence.

顔写真は正面・無帽・無背景・髪の毛が目にかからないように撮影してください。サングラス着用は不可
住民票は本籍の記載してあるもの。本籍欄が「省略」と記載してあるものは無効です。また、マイナンバーは必要ありません。マイナンバーが記載された住民票をお取りになった場合は、マイナンバーの部分をマジック等で塗りつぶし、読めないようにしてからお送りください。
住民票は後ほど免許証作成時に現物が必要になりますので郵送してください。
コピー類はスキャナで撮るか、記載されている字が読み取れるようにカメラで撮影しください。
自動車運転免許証コピーはボート免許証再交付や受験申請時に住民票の取得が間に合わない場合の本人確認用です。とくに受験申請の場合は試験日までに本籍記載の住民票と差し替えてください。試験日までに本籍/国籍が住民票により確認できない場合は受験できませんのでご注意ください。また免許証作成時には実物の住民票が必要になりますので、後ほど現物を郵送してください。

有資格者が進級や別種の免許を取得する場合は住所、氏名、本籍の都道府県名に変更が無い場合は住民票に替えて小型船舶操縦免許証(ボート免許)のコピーでけっこうです。新免許証作成時に現物は運輸局に返納します。

「見つかりませんでした。」と表示された場合にはお手数ですが、印刷／記入／捺印した現物を郵送してください。
